The fee for the initial visit is $250. We require a non-refundable $50 deposit at the time of booking your appointment. The $50 deposit will go towards your office visit and be deducted out of the total cost of your visit. The deposit is to secure your appointment. The cost of your appointment or the deposit can be paid over the phone by credit card or in person with cash or credit card. No checks please.
The fee for the 45 to 60 day follow up visit is $100, also payable in advance. This is highly recommended, but not mandatory. This visit is used to discuss your progress and any changes you need in your treatment regimen.
The fee for the state mandatory 210 day renewal visit is $250, and is payable in advance by phone or in person.
We do not bill insurance or third party payers. We will be happy to give you a receipt for your payment if you feel you can get reimbursement from any third party payer.
There are no monthly fees or other charges.
